UNITED HOCKEY WAIVER

Release of Liability, Waiver of Claims and Indemnity Agreement

IMPORTANT: PLEASE READ CAREFULLY BEFORE SIGNING

The undersigned consents to the participant participating in all United Hockey programs, including outdoor recreation and athletic programs, and acknowledges that such participation involves risks and hazards incidental thereto, including injury, illness and quarantine arising from epidemic or pandemic, including from COVID-19, death, property damage and loss resulting therefrom. The undersigned freely accepts and fully assumes all such risks and hazards. In consideration of United Hockey agreeing to the undersigned’s participation in United Hockey’s programs and for other good and valuable consideration, the receipt and sufficiency of which is acknowledged, the undersigned on his or her own behalf agrees to waive any and all claims the undersigned or their personal representatives and next of kin have or in the future shall have against United Hockey, its owners, employees, contractors and agents (the “Releasees”) and to release the Releasees from any and all liability for any loss, damage, expense, illness or injury, including illness or injuries causing death the undersigned may suffer as a result of participation in United Hockey programs due to any cause whatsoever, including negligence, breach of contract or breach of any statutory or other duty of care on the part of the Releasees, and the undersigned agrees to indemnify and save harmless the Releasees of and from any liability or claim arising from the participation in United Hockey programs.
